Patient Care Portfolio (Safer Nursing Care Tool)
The AUKUH Patient Care Portfolio was formally launched at the CNO Summit on 1 November 2007. Further work to develop the Patient Care Portfolio was then carried out by the NHS Institute and later, The Shelford Group. This resource then became known as the Safer Nursing Care Tool.
Following extensive review of the tool, its definitions and multipliers, commissioned by the Shelford Group's Chief Nurses' Sub-Group, it is now relaunched as The Safer Nursing Care Tool.
The Safer Nursing Care Tool is made up of 2 elements:
This element comprises of a decision matrix based upon the classification of levels of critical care patients (Comprehensive Critical Care, DH 2000). This to used to determine the level of acuity/dependency of all patients. When this is aligned to the evidence based nurse staffing multiplier, it allows recommendations for nurse staffing required per ward based on the actual needs of those patients.
Nurse Sensitive Indicators such as infection rates, complaints, pressure ulcers and falls, are monitored to ensure that the staffing levels determined in Element 1 are enabling the delivery of patient outcomes we aim to achieve. Within the SNCT these data are converted into a rate per 1000 occupied bed days thus allowing comparison across wards.
